Two Treatments That Work

Which One Is Right For You?

There are only two medications in this category worth talking about, and both are extensively studied. Which is better depends on your goal, your history and your budget - here is how to think about it in plain language. Your clinician makes the final call with you.

The proven starting point

Semaglutide

The most-studied medication in this category, and the more affordable way in. Strong, steady appetite control, and the deepest research record for benefits beyond the scale.

Choose this if you…
  • Want the option with the longest track record
  • Would rather start gently and build up
  • Prefer a tablet to a weekly injection
  • Care about heart, kidney or liver health alongside weight
  • Want the lowest monthly cost to stay consistent
$129-$229 per 4 weeks, member price, all in
The strongest option

Tirzepatide

Works on two hunger pathways instead of one. In the only head-to-head trial of the branded products, it out-performed semaglutide by a wide margin.

Choose this if you…
  • Have a larger goal and want the strongest average result
  • Have already stalled on semaglutide
  • Struggle with sleep apnea alongside your weight
  • Want the shortest path to a visible change
  • Are comfortable with a once-weekly injection
$129-$289 per 4 weeks, member price, all in

You are not locked in.

Plenty of members start on semaglutide because it costs less, then move to tirzepatide if progress stalls. Others start at a microdose to see how their body handles it before committing to a full dose. Switching is a conversation with your clinician, not a penalty or a new sign-up.

One Price, All In

The Price You See Is The Price You Pay

Most online weight-loss companies advertise a low number and build the margin back somewhere else, a consultation fee, a charge every time your dose goes up, a "premium" tier for the support you assumed was included. We priced this the other way round.

The usual online playbook

Where the extras hide

  • A separate consultation or "medical evaluation" fee before you get a price
  • A higher charge each time your dose increases, so the cost climbs as you progress
  • Follow-up visits billed individually, or locked behind an upgraded plan
  • Lab work not included, you pay out of pocket, or simply go without it
  • An introductory rate that jumps after the first month or two
  • Long prepaid commitments that are difficult to cancel
  • Support that is email-only unless you upgrade
How OptimalMD works

What your price covers

  • Your clinician consultation is $0 - included with membership
  • Every dose level costs the same. Titrating up never raises your price
  • Follow-ups and dose adjustments are included, not billed per visit
  • 3,900+ lab tests at $0 through LabCorp when ordered through OptimalMD
  • The same price every month. No introductory rate that quietly resets
  • No contract. Cancel any time, no cancellation fee
  • Unlimited virtual care and a care team you can message, included

Your medication price covers the medication and the medical care around it. The only thing you add is your membership, which is also what unlocks the $50 discount, the $0 labs, and everything else on this list.

Why most members start here

This is full therapeutic dosing on the molecule with the deepest research record of any GLP-1. You begin at a low dose and step up roughly every four weeks, so your body has time to adapt, which is exactly why most side effects stay mild. One injection a week, and the appetite signal holds steady between doses.

For anyone who would rather not inject

Same active ingredient, different delivery. A dissolvable tablet taken once a day instead of a weekly shot - nothing to refrigerate, nothing to pack in a cooler, and no needle at all. The trade-off is that it has to be taken consistently every morning on an empty stomach, so it suits people with a steady daily routine.

The strongest option we offer

Tirzepatide works on two gut-hormone receptors instead of one, and in the only head-to-head trial of the branded products it out-performed semaglutide by a wide margin. It is also the molecule behind the first medication ever FDA-approved for obstructive sleep apnea. It costs the most, and for members with a large goal, a plateau, or apnea alongside their weight, it is usually worth it.

The trial figures shown come from branded, FDA-approved medications at full therapeutic doses, not from compounded products, and not from microdose regimens, which have not been studied in clinical trials. Compounded medications are not FDA-approved and are not generic or equivalent versions of any brand. Prices shown are the OptimalMD member price per 4-week supply, already reflecting the $50 member discount off the current market price. Membership is verified at checkout. Pricing is subject to change and does not include applicable shipping or state taxes. All medications require a prescription issued by a licensed clinician following a medical evaluation - completing an intake or a purchase does not guarantee that a prescription will be written.

The part almost nobody else includes

The Lab Work That Tells You It's Actually Working

The scale is the crudest measure of what these medications do. The meaningful changes show up in your blood - insulin resistance easing, inflammation falling, lipids improving, thyroid and hormones settling. Most online GLP-1 sellers never order a single test. Your OptimalMD membership includes 3,900+ diagnostic tests at $0, so you and your clinician can actually see what is happening.

Metabolic health

The core of what a GLP-1 is doing. These show whether your body is handling sugar and insulin better, often before the scale moves much at all.

Commonly ordered
A1c, fasting glucose, fasting insulin, insulin resistance markers, comprehensive metabolic panel

Cardiovascular risk

Weight loss changes your lipid picture. These are the numbers your doctor would use to judge your long-term heart risk, not just your cholesterol total.

Commonly ordered
Full lipid panel, ApoB, Lp(a), triglycerides, HDL/LDL particle detail

Inflammation

Chronic inflammation is one of the quiet drivers behind fatigue, joint pain and cardiovascular risk. It is also one of the things that tends to move most.

Commonly ordered
hs-CRP, ferritin, ESR and related inflammatory markers

Hormones & thyroid

Thyroid and sex hormones shape how you lose weight, how you keep muscle, and how you feel doing it. Worth knowing before you start, not after you stall.

Commonly ordered
TSH and thyroid panel, testosterone, estradiol, DHEA-S, cortisol

Nutrients

Eating substantially less means taking in less of everything. Checking these is how you avoid trading weight loss for deficiency, fatigue or hair thinning.

Commonly ordered
Vitamin D, B12, folate, iron and ferritin, magnesium

Liver & kidney

Two organs that benefit most from losing visceral fat, and two your clinician wants to watch while you are on treatment. Both are simple blood tests.

Commonly ordered
Liver enzymes (ALT, AST, GGT), kidney function and eGFR, urine albumin
Step 01

Assess

Baseline labs before or soon after you start, so there is something real to measure against.

Step 02

Optimize

Your clinician sets the molecule and dose against what your results actually show.

Step 03

Support

Protein, training, sleep and any deficiency corrected, so you lose fat, not muscle.

Step 04

Sustain

Re-test as you go. Re-testing is $0 too, so there is no reason to fly blind.

Lab tests are included at $0 when ordered through OptimalMD and completed at LabCorp; results are typically available within 72 hours. Quest and regional labs are available at discounted member rates rather than $0. Which tests are appropriate for you is a clinical decision made by your provider.

Included With Every Plan

Coaching Is Included, Not An Upsell

Every OptimalMD medical weight loss member gets Care Coaching at no extra charge. Most programs either leave it out or bill for it separately. Yours is part of the membership from day one.

Your first session is booked at signup

It is scheduled when you join the program, for roughly your first two weeks of treatment, around your first one or two doses.

Every two weeks, or your own pace

Each follow-up is booked at the end of the session before it, two weeks later, unless you want a different rhythm. You can pause at any point.

Delivered by Certified Medical Assistants

Coaching is medically centered on your treatment and on the non-medication goals you set for yourself, not on a generic script.

A care plan that follows you

Your plan is built with your coach, evolves over time, and is written into your medical record, so you and your doctors always see the current version.

What actually happens in a session

Every session has three parts: a review of your program, covering side effects, how you are getting on with the medication, and progress on weight and biomarkers; a look back at your care plan and the goals you set, updated as things change; and one new coaching module, and how it fits the plan you already have. Afterwards you get a short, easy summary of the module you covered.

Modules change over time. They currently include managing and reducing side effects on a GLP-1, several on nutrition as it applies to GLP-1 patients, lifestyle and exercise from a medical weight loss angle, emotional eating and ways to handle it, stress and how it interacts with weight loss, sleep, and social situations such as holidays, entertaining and eating out.

The concierge and care coach team will reach out to keep the rhythm going, always at your discretion and to your preference, including if you want to pause.

What it is not. Care Coaching does not include registered dietitian services, prescriptive diets or limits on eating, mental health therapy, or other specialized professional services.

Why People Actually Start

It Was Never Really About the Number

When we ask members what they want, almost nobody leads with a weight. They lead with something they have lost, or something they are afraid of losing. These are the reasons we hear most.

Getting ahead of a diagnosis

A prediabetes result, a blood pressure reading, a family history that finally feels close. Wanting to change direction before it becomes permanent.

Quieting the food noise

The constant negotiation with food that runs in the background all day. Many people describe this going quiet as the change that surprised them most.

Keeping up

With your kids, your grandkids, the hike everyone else is doing. Being the person who comes along rather than the one who waits at the bottom.

Knees, hips and stairs

Carrying less weight is the one thing that reliably takes load off the joints. For a lot of people this matters more than anything on the scale.

Feeling like yourself again

The clothes in the back of the closet. The photo you didn't want taken. Walking into a room without doing the maths about how you look.

Energy that lasts past 6pm

Getting to the end of the day with something left. Sleeping properly and waking up like it counted for something.

A word about expectations.

These are the goals members bring us, not results we can promise you. How much weight you lose, and what else changes with it, depends on your starting point, your medical history, your dose, and what you do alongside the medication. Some people lose a great deal. Some lose little. Your clinician will be straight with you about what is realistic for your situation.

Why Membership Pays for Itself

Getting Started

From Question to Front Door

No waiting rooms, no prior authorizations, no insurance claim attached to your name. Just a clinician, a plan, and a package.

Step 01
1

Take the intake

A short online health history, your goals, your medications, your conditions. Five minutes, on your phone, whenever you have a moment.

Step 02
2

Meet your clinician

A US-licensed provider reviews everything, talks through the options, and decides whether a GLP-1 is appropriate for you, and if so, which one.

Step 03
3

Delivered discreetly

If prescribed, your medication ships from a licensed pharmacy directly to you, with tracking and everything you need to start safely.

Step 04
4

Stay supported

Ongoing check-ins, dose adjustments when you need them, and a care team you can message. Cancel any time, without a fee.

Start the conversation. The rest follows.

Five minutes of health history is all it takes to find out whether a GLP-1 is right for you, and which one. If it isn't, your clinician will tell you that too.

Step 2 of 2

Want to know exactly how it works before you commit?

Everything above is what the program is and what it costs. Step 2 is the evidence underneath it, the physiology of how these medications act on your gut, pancreas and brain, exactly which medication you would be taking and how it relates to the brand names, what the large clinical trials found, and an honest account of the side effects.

What you should know before you start

These compounded medications are not FDA-approved. Compounded drugs are prepared by a licensed pharmacy for an individual patient based on a prescription. The FDA does not review compounded medications for safety, effectiveness or quality before they are marketed. Compounded semaglutide and tirzepatide are not generic, equivalent or substitute versions of Ozempic®, Wegovy®, Rybelsus®, Mounjaro® or Zepbound®.

Any clinical results you have seen for these molecules come from branded, FDA-approved medications, not from compounded formulations. Individual results vary and no outcome is guaranteed. These medications are available only by prescription and only after evaluation by a licensed clinician; completing an intake or a purchase does not guarantee that a prescription will be issued. They carry a boxed warning regarding thyroid C-cell tumors and are not suitable for everyone.

Lab tests are included at $0 when ordered through OptimalMD and completed at LabCorp; Quest and regional labs are available at discounted member rates.
